Update: Quick research tells me that the bad behaviors was due to the use of the "clone" button.
- Tip: Avoid using the CLONE button when building questions in a quiz in LearnDash.

Thank you for pointing me to Ryk's comment. It does look like an attempt to clone Questions with a third party plugin is involved in this behavior. I apologize for not catching this earlier.
WordPress or LearnDash does not support cloning content, and we wouldn't be keen to recommend using a third party plugin to try and achieve this because of the complexities involved at the database level.
Instead, you could consider using our Shared Quiz Questions feature as mentioned on our support guide here: https://www.learndash.com/support/docs/core/quizzes/global-settings/#quiz-builder
Please try out this feature on a test site first, since it shouldn't be disabled right away — questions linked to multiple Quizzes will lose their association.
